LOCSU has arranged a package of insurances for its Primary Eyecare Companies comprising:
- Medical Malpractice
- Public and Products Liability
- Employers’ Liability
- Directors’ and Officers’ Liability
- Crisis containment
To apply for the insurance, an application form will need to be completed, including any regulator sanctions. The insurance does not need to be taken up until there is a service contract in prospect.
After your first-time application, a renewal form is issued each year you have insurance to update any details. This form is issued by us and once you’ve returned it to us, we then forward it to the insurer, with at least 2 weeks before the renewal date to ensure continual cover.
